Referral & assessment for our programs

Referral & assessment to participate in our programs is initially via the phone and any one can call to make a referral or get some information about availability and entry to services. Assessment by staff from Lyndon must be completed before admission to a program.

A pre-admission assessment will ask about;

  • What, how much, how often, when, for how long drugs and alcohol have been used

Follow-up and on-going assessments include;

  • Benefits and problems related to substance use
  • Previous experience of cutting down or abstinence
  • Aims for reducing or stopping use now
  • Concerns about change
  • Risks of harm related to withdrawal, intoxication and mental state
  • Related factors – culture, gender, mental health, physical health, family, caring, work, environmental and financial

Lyndon Live-in Programs

To book a place Medicare card number, Centrelink reference number (CRN) and one additional form of ID is required.

Methadone and Buprenorphine Detox

Clients wishing to detox off methadone must be on 20mgs or less and buprenorphine 6mgs or less. At the time of assessment, contact details of the clinic prescribing these drugs is required.

We are also able to maintain a person on methadone/buprenorphine/suboxone (selective detox) and detox them from other substances.

Mental Health Issues

Clients who have a history of mental health issues may be required to fax through copies of their reports as supporting documentation for their assessment- 02 6361 7400

Criminal Offences

Clients who have a history of criminal convictions issues may be required to fax through copies of their reports as supporting documentation for their assessment- 1300 LYNDON

Where offences are of aggravated/sexual violence in nature, Lyndon reserves the right to decline admission.

Only clients on the MERIT (Magistrates Early Release into Treatment) program will be assessed whilst in custody.
